How to claim

Please follow these simple steps to ensure a smooth and efficient claims process:

1. Funeral Benefit Claim

Along with the completed claim form, you'll need to submit the following documents:

  • Certified copy of claimant ID/Omang
  • Certified death certificate
  • Copy of marriage certificate (if applicable)
  • Birth certificate for minor (if applicable)
  • Proof of bank details
  • Payslip or bank statement

Kindly submit all required documents and the completed claim form to lifeclaims@hollard.co.bw. Please mention 'Funeral Benefit Claim' in the email subject along with your applicant ID number. 

2. Cashback Benefit Claim

Along with the completed claim form, you'll need to provide the following documents:

  • Certified copy of claimant ID/Omang
  • Proof of bank details
  • Payslip or bank statement

Kindly submit these documents along with the completed claim form to lifeclaims@hollard.co.bw. Please use 'Cashback Benefit' along with your applicant ID number as the email subject.

3. Premium Refund Claim

Along with the completed claim form, you'll need to provide the following documents:

  • Certified copy of claimant ID/Omang
  • Proof of bank details
  • Payslip or bank statement

Kindly submit these documents along with the completed claim form to customerservice@hollard.co.bw. Mention 'Premium Refund' in the email subject along with your applicant ID number.
